New healthcare support program launched in Grey Bruce

A new healthcare coordination platform has been launched in Grey and Bruce Counties at no cost to residents with a goal to better coordinate care.

Brightshores Health System says Serefin Health, in partnership with Brightshores and the Grey-Bruce Ontario Health Team (OHT), has launched the MyCareSteps program, which Brightshores describes as a "proof-of-concept program, to support care coordination for residents across Grey and Bruce counties and to inform future approaches to coordinated care delivery."

This program connects patients to "nurse care coordinators" who will then provide virtual support for patients, including helping individuals and their families understand care plans, self-manage health conditions between any appointments and connect with health and community services.

The program began on April 1 and is offered at no cost to 175,000 residents spread across Bruce and Grey. Residents can access MyCareSteps through referral from a primary care provider, a hospital-to-home referral after discharge, or by self-referral.
