Erie Shores HealthCare is introducing a new smartphone app to connect patients and their healthcare providers to manage and coordinate care within the hospital and at home.
The hospital in Leamington is the first to provide the service in Essex County.
Already, 30 health systems across the country use CoHealth, including Chatham-Kent Health Alliance, North York General, and Halton Healthcare.
The Maternal-Newborn Services and Rehabilitation Program first launched the app on Thursday. In August, Erie Shores HealthCare will add inpatient services, the surgical program, diagnostic imaging, and laboratory services. The hospital will add the Emergency Department and other areas of the hospital in September.
"CoHealth, born out of a patient experience, was created with the vision to connect the dots in health care," said CoHealth CEO Zack Fisch. "To us, that means being able to provide patients with a single entry point to manage their healthcare experience; whether it's accessing educational information to support understanding or managing medications on a day-to-day basis."
Signs will be posted in the hospital over the coming weeks.
Patients will also receive cards explaining the new app and encouraging them to download it.
Leamington's hospital has 58 inpatient beds and a 24-hour emergency department, ambulatory care, outpatient diagnostic and therapeutic services, and intensive care unit.
